Providers with any health care provider taxonomy code listed as hematologist (207 RH0000 X, 207 RH0003 X, or 2080 P0207 X) were categorized as a hematologist; nephrologists were categorized as such if they had any health care provider taxonomy code listed as a nephrologist (207 RN0300 X).

Nephrologists have called for the integration of other disciplines to enhance AKI survivor care delivery capacity [12], including primary care providers (PCP) to evaluate laboratory tests and perform a kidney function assessment early in the postdischarge period [13].
To address these concerns, we developed the multidisciplinary AKI in Care Transitions (ACT) program, which embeds post-AKI care in the patient’s medical home, primary care.
