Hematologist and nephrologist encounters were identified using the National Provider Identifier of the rendering provider, as listed in the Medicaid claims records [21]. Providers with any health care provider taxonomy code listed as hematologist (207 RH0000 X, 207 RH0003 X, or 2080 P0207 X) were categorized as a hematologist; nephrologists were categorized as such if they had any health care provider taxonomy code listed as a nephrologist (207 RN0300 X).

Despite this increased need, there is only 1 nephrologist for every 593,000 population compared with every 44,000 population in Bangkok [3]. In 2008, the Thai government adopted the Peritoneal Dialysis First policy for renal replacement therapy under its universal health care coverage scheme, increasing access in low-resource settings, with approximately 21% of patients on peritoneal dialysis (PD) in northeast Thailand [4].
