For the consistency-based approach, we followed the best practices established in prior work [24] by sampling the LLM multiple times (number of samples=5) with high temperature (T=0.8; high randomness in generated responses) and used a majority vote weighted by the confidence of each response to determine the final deprescribing recommendation.

For group comparisons, we used the chi-square or Mc Nemar test for nominal variables and the t test for ordinally scaled data (2-sided, α=.05). All calculations were performed with SPSS (version 28.0; IBM Corp).
Figure 1 shows the CONSORT (Consolidated Standards of Reporting Trials) flow diagram of this underlying RCT (Multimedia Appendix 2) [19]. The average time period between T0 and T1 was 13.4 months (SD 2.9 months; range 6.1-34 months).
