In a study by Yoon et al [12], 69.8% (2051/2570) of cases had initial vancomycin trough concentrations outside the therapeutic range (10-20 mg/L). According to the latest clinical guidelines on TDM for vancomycin, the recommended dosage for serious MRSA infections is 15-20 mg/kg every 8-12 hours [6]. While the vancomycin package insert provides renal function-based dosing guidelines, in real-world clinical practice, initial doses are often determined based on the clinician’s experience [13,14].
