Our study (currently under review) further investigated the manifestations of information distortion in ENRs through a qualitative approach (Jianan Wang, BS, unpublished data, January 2025). The findings revealed that information distortion permeates various aspects of ENRs, including assessments, care planning, interventions, incident management, and billing. This is particularly concerning as certain decision-making processes cannot tolerate inaccurate information, which may lead to severe consequences.
